Waiver / Entry Forms: All big bike riders under the age of 18 MUST either have BOTH parents present to sign a per-event waiver OR complete the AMA annual waiver process in order to race an AMRA event. Note: * must be printed in color, double-sided * must be signed by both parents * needed for everyone under 18  All big bike riders under the age of 18 MUST either have BOTH parents present to sign a per-event waiver OR complete the AMA annual waiver process in order to race an AMRA event. Note: * must be printed in color, double-sided * must be signed by both parents * must be notarized * must be sent in to AMA per instructions on form * needed for everyone under 18   All mini bike series riders MUST have this waiver signed, notarized and given to AMRA for our files before they can race an AMRA event. Note: • Must be printed in color and double sided (no stapled together pages) • Original Color copy of the AMRA Minor Waiver, not the AMA Waiver (no black and white copies allowed) • Both parents signature are required. • Minor must sign page # 2 • If parents are divorced, both parents signatures are required none-the-less. • If one parent, check the box as "sole legal guardian" • Form MUST be notarized, no exceptions • Turn completed, notarized form in at the next AMRA event • Please, do NOT mail, e-mail, scan or electronic sign these forms  |
